A tourniquet is a hemostatic device used to manage severe, life-threatening extremity bleeding. It is primarily applicable for injuries on the arm or leg that involve strong, spurting, pulsating, or large volumes of bleeding, where stopping the blood loss quickly can be life-saving. Such situations may occur during traffic accidents, workplace accidents, cuts, crushing or tearing injuries, near-amputations, or any injury where traditional direct pressure or pressure dressings are insufficient. The principle behind the device is to constrict the blood vessels with sufficient force above the site of injury on the affected limb, thereby reducing or completely stopping blood flow to the injured area. A tourniquet typically consists of a strong strap, a fastening system, and a windlass rod. The strap is placed around the injured limb, and then tightened using the windlass rod until the intense bleeding stops. Following this, the device must be locked in place to prevent the tension from loosening. It is important to emphasize that a tourniquet is not meant for general wounds, minor bleeding, or simple abrasions. Its use is specifically justified for severe, life-threatening extremity bleeding. Improper application can cause pain, tissue damage, or other complications; however, in truly critical bleeding scenarios, its rapid and professional use can save lives. Once applied, the device should not be loosened or removed without justification, and emergency services must be called immediately for the victim via the 112 emergency number.
